Every application shall contain the following: A. A description of the premises by street number or otherwise. B. The name, residence and business address of each owner; if the owner is a corporation, the name and address of such corporation and the names, residences and business addresses of every officer thereof. If the applicant is a lessee or sublessee, a confirmed copy of the lease is to be attached and, if oral, the material terms of such lease shall be set forth. C. If the applicant is a corporation or a partnership or other business entity, or if the applicant is a nonresident, such applicant shall designate the name and address of an individual residing within the City upon whom any notice or order of the Commissioner may be served and such individual's written consent thereto. A diagram showing all rooms to be used for single-room occupancy, designated by letters for identification, and each bathroom facility, together with the dimensions and aggregate floor area of each room. E. The maximum number of occupants to be permitted to occupy each room and the maximum number of occupants to be permitted to occupy the entire rooming house. F. The zoning district in which the premises sought to be licensed are located.
Section 211-4 of Mount Vernon, N.Y., Code of Ordinances ch. 211 establishes the required contents of an application to operate a rooming house under single-room occupancy licensing. The application must identify the property, its owners or corporate officers, a local agent for service of process if the owner is a nonresident or business entity, a diagram of all rooms and bathrooms with dimensions, occupancy limits for each room and the whole building, and the applicable zoning district. If the applicant holds a lease rather than owns the property outright, a copy of that lease or its material terms must also be included.

Operators seeking a rooming house license under Mount Vernon Code § 211-4 are generally expected to compile a complete application package before submission. A compliant application typically includes a detailed floor diagram with room letters, dimensions, and floor areas; stated maximum occupancy figures for each room and the entire facility; and, for corporate owners or nonresidents, the name and address of a local individual authorized to accept official notices. Attaching a confirmed copy of any lease—or documenting its material oral terms—and noting the property's zoning district are also standard parts of a complete filing.

Section 211-4 of Mount Vernon's ch. 211 requires that rooming house operators disclose specific information about the property, its ownership, and its occupancy limits as part of the licensing process. Tenants living in single-room occupancy housing may find this provision relevant when verifying whether a facility is properly licensed, since the application on file should reflect the maximum number of occupants permitted per room and for the building as a whole. Those with concerns about whether a rooming house meets these disclosure requirements can inquire with the relevant City office, consult a local tenant-rights organization, or review publicly available licensing records.
